We believe that by working closely with our referring dentists, our patients receive exceptional care, great communication, and the best possible care for their dental health and well-being.
Refer Patients Online
By referring patients through the TDO Sign-in area, it is possible for our referring doctors to track patient activity, read reports & updates, and more.
New Referring Doctors: Register Online Now
First time registration is quick and easy. Just fill in basic information about your practice and contact numbers, choose a user name and a password, and you are ready to go.
STEP 1: Go to Online Registration Page
STEP 2: Fill out basic information about you, your practice, and contact information. You will be asked to choose a username & password… Click the SUBMIT button.
STEP 3: A verification email will be sent to the address you used to register. That’s it. You’re ready to go.
STEP 4: Go to Referring Doctors Area to Login. Login can always be found at the top of the sidebar. REFERRING DOCTOR’S LOGIN
Returning Doctors: Sign In Now
After your initial registration, you can refer patients simply and easily by using the link in the sidebar menu. You may enter your username & password, then select REFER PATIENT.
After you have filled out the necessary patient information, made notations of teeth that are needing treatment, and teeth which may need to be evaluated, please be sure to fill out any special considerations for this patient or other instructions in the provided text box. By clicking the SUBMIT DATA button, the secure form will be available for review by our office.

Order Referral Pads (50 forms per pad)
If you would like to keep referral pads on hand in your office, just let us know! Each pad contains 50 referral forms (with convenient map & directions on the reverse), and each form uses carbonless copies, so referring doctors can keep a copy of referral notes in their patient’s file.
